Both you and/or your eligible spouse had an opportunity to earn a $15 per paycheck premium discount if you completed the actions below by August 30, 2024. Note: You must enroll in a Cintas Medical Plan for 2025 to receive the premium discounts shown below. If you were hired on or after July 13, 2024, or you added your spouse on your coverage on or after July 13, 2024, you will automatically get this LiveWell premium discount for 2025.

*Both the Biometric Screening and the Health Assessment must be completed to receive the full weekly premium discount in 2025. If you only completed the Biometric Screening, you will receive a $10/week premium discount in 2025. If you only completed the Health Assessment, you will receive no premium discount in 2025.

Virtual Second Opinion
Partners in a Cintas medical plan can get a free second opinion on a health care issue by speaking with a qualified health care professional through My Medical Ally. Call 888.361.3944 Monday – Friday, 8 am to 8 pm CST.

You and your benefits-eligible family members may receive up to five in-person counseling sessions (session maximums apply) with a licensed clinician to address issues such as marriage and family problems, substance abuse, stress, anxiety or other behavioral health concerns. - Available 24/7 in all states and can choose U.S. board-certified doctor based on profile.
